Health Services Coordinator:

The Health Services Coordinator will assist the Health Services Department meet the medical needs of clients including accompanying clients to medical appointments and maintain medical data (on line and hard copy).

Essential Functions:

1.          Maintain medical database per established procedures and standards; perform data entry.

2.          Prepare files for appointments, noting in advance the location of client and doctor.

3.          Schedule appointments as required.

4.          Take residents to Doctor appointments as well as other health care professional appointments.

5.          Maintain medical hard files.

6.          Respond to drop-in health concerns, injuries, illnesses, and emergencies.

7.          Maintain cleanliness of the Health Services offices and treatment areas.

8.          Communicates routine client health and behavior information with residences and administration.

9.          Setting up and administering client medications; record/log as required by Title 17 and 22.

10.       Triage phone calls concerning client medical concerns, contact appropriate professionals.

11.       Communicate professionally by phone with client’s families, Cedars’ staff and health care professionals.

12.       Communicate with pharmacies to ensure proper medication accuracy and documentation.

  • Use email and computer.
  • Require CPR and First Aid certifications or ability to acquire it.
  • Clean DMV record.
  • Valid CA driver license.
  • Pass TB test.
  • Pass criminal background check.
  • Pass drug test.
  • Pass physical and meet physical requirements of job.
  • Must pass both DSP (Direct Service Provider) I and II, within first 12 and 24 months of hire, respectively
  • Communicate effectively in English, both written and verbally.
  • Ability to work weekends.
